Field Guide to Understanding Human Error

Sidney Dekker - 2002
You think you can solve your human error problem by telling people to be more careful, by reprimanding the miscreants, by issuing a new rule or procedure. These are all expressions of 'The Bad Apple Theory', where you believe your system is basically safe if it were not for those few unreliable people in it. This old view of human error is increasingly outdated and will lead you nowhere. The new view, in contrast, understands that a human error problem is actually an organizational problem. Finding a 'human error' by any other name, or by any other human, is only the beginning of your journey, not a convenient conclusion. The new view recognizes that systems are inherent trade-offs between safety and other pressures (for example: production). People need to create safety through practice, at all levels of an organization. Breaking new ground beyond its successful predecessor, The Field Guide to Understanding Human Error guides you through the traps and misconceptions of the old view. It explains how to avoid the hindsight bias, to zoom out from the people closest in time and place to the mishap, and resist the temptation of counterfactual reasoning and judgmental language. But it also helps you look forward. It suggests how to apply the new view in building your safety department, handling questions about accountability, and constructing meaningful countermeasures. It even helps you in getting your organization to adopt the new view and improve its learning from failure. So if you are faced by a human error problem, abandon the fallacy of a quick fix. Read this book.

Remember Solomon's prayer for wisdom? He received it-and wealth beyond his imagination as well. And how about Hannah's desire for a child? God honored her prayer; her son Samuel became the greatest judge in Israel's history. Pivotal Praying uses these examples and others to illustrate the power of effective-and ineffective-praying. For those seeking to enlarge their prayer vision and alter their circumstances for God's glory, Pivotal Praying is an ideal resource.

Trust in Schools: A Core Resource for Improvement: A Core Resource for Improvement

Anthony S. Bryk - 2002
The rhetoric of standards and vouchers has occupied center stage, polarizing public opinion and affording little room for reflection on the intangible conditions that make for good schools. Trust in Schools engages this debate with a compelling examination of the importance of social relationships in the successful implementation of school reform.Over the course of three years, Bryk and Schneider, together with a diverse team of other researchers and school practitioners, studied reform in twelve Chicago elementary schools. Each school was undergoing extensive reorganization in response to the Chicago School Reform Act of 1988, which called for greater involvement of parents and local community leaders in their neighborhood schools. Drawing on years longitudinal survey and achievement data, as well as in-depth interviews with principals, teachers, parents, and local community leaders, the authors develop a thorough account of how effective social relationships—which they term relational trust—can serve as a prime resource for school improvement. Using case studies of the network of relationships that make up the school community, Bryk and Schneider examine how the myriad social exchanges that make up daily life in a school community generate, or fail to generate, a successful educational environment. The personal dynamics among teachers, students, and their parents, for example, influence whether students regularly attend school and sustain their efforts in the difficult task of learning. In schools characterized by high relational trust, educators were more likely to experiment with new practices and work together with parents to advance improvements. As a result, these schools were also more likely to demonstrate marked gains in student learning. In contrast, schools with weak trust relations saw virtually no improvement in their reading or mathematics scores.Trust in Schools demonstrates convincingly that the quality of social relationships operating in and around schools is central to their functioning, and strongly predicts positive student outcomes. This book offer insights into how trust can be built and sustained in school communities, and identifies some features of public school systems that can impede such development. Bryk and Schneider show how a broad base of trust across a school community can provide a critical resource as education professional and parents embark on major school reforms. Social Power and the CEO: Leadership and Trust in a Sustainable Free Enterprise System

Elliott Jaques - 2002
It affects the quality of economic life, but also our personal and social lives. Equally strong is its impact on the sustainability of a free enterprise system. Psychoanalyst, teacher, and management consultant, Elliott Jaques argues that great as this power is, it is being squandered, not because of what managers do but because of what they don't know. Serious misconceptions about managerial leadership--and equally serious misunderstandings of people--abound. Jaques argues that the problems inherent in the way management is practiced are attributable to gravely dysfunctional systems of managerial leadership, systems that have evolved over the years and are now, despite their ineffectualities, taken for granted. He shows how the CEO class will determine the future of free enterprise democracy, but how massive misconceptions about human behavior have undermined its capability for leadership. It's the managerial leadership systems that need changing, he maintains, not the people who implement them. The result of more than a half century of thought, observation, analysis and experimentation, Jaques' book offers a totally new and creative system of managerial leadership and a unique system of managerial organization. In doing so, it becomes essential reading for academics, students, consultants, top management, and executives on the way up throughout the public and private sectors.Jaques argues that the problem of achieving effective and sustainable managerial organizations does not lie in poor decision making, interpersonal stress, lack of innovation, greedy self-interest, and other ills and defects, although they certainly play a part. The art of management, somewhat like alchemy, is not securely grounded in science. This is its most severe weakness, and the reason why organizations have always been badly managed. He cites examples to show that management tends to blame its shortcomings and inefficiencies on other people, yet there is no research to prove that the source of managerial failure is really to be found there. People strive to do their best, he has found, but they are stymied by poor organization and systems that pitch people into conflict with each other. Jaques looks at these systems closely, particularly our current systems of compensation, and itemizes his findings, showing how the the same problems are to be found throughout industry, public service, health and services organizations, and less surprisingly perhaps, the military. He then lays out the ways in which a new system of managing operations and organizations could work, shows that it is already working in certain selected places, and provides convincing evidence that his assertions and recommendations have much to offer, as we continue to search for better, more efficient, and more productive, profitable organizations.

Pat Williams - 2002
Examining the paradoxes that made Christ an exceptional leader, he then offers fascinating and instructive examples that make those principles relevant today. Leading Beyond the Walls

Adam Hamilton - 2002
It is a matter of heart and soul, of devoting the whole self to the vision God gives for the congregation in which one serves. Yet neither is it purely intuitive; it requires hard, careful thinking about the directions and details of the path down which God calls. When Adam Hamilton became pastor of the United Methodist Church of the Resurrection, its membership consisted of himself and his family. Ten years later the church averages between five and six thousand worshipers per weekend. Throughout this remarkable period, Hamilton learned many serious lessons about both the broad visions and the specific details of pastoral leadership.Bringing a depth of analytical skills often lacking in visionary leaders, in this book he goes beyond simply telling the story of Church of the Resurrection. He shares the questions that he learned to ask about the largely unchurched population to which Church of the Resurrection has reached out. Further, he demonstrates what he learned by listening to the answers to these questions, and how doing so has made possible a number of strategically crucial decisions the church has made. One of those crucial decisions was to make more traditional forms of worship and praise the center of the congregation's life. The result is that the example of Church of the Resurrection offers pastors and church leaders (especially those in mainline denominations) the realization that they need not completely change their liturgical and theological identity in order to reach out to the unchurched.Drawing on his own experience, as well as the detailed research on the characteristics of highly successful congregations he undertook during a sabbatical leave, Hamilton offers pastors and other church leaders solid, substantive thinking on steps that congregations can take to become centers of vibrant outreach and mission. Rudolph W. Giuliani - 2002
    11. Brotherhood gives a glimpse into the firefighters’ lives after the tragedy.... A lively introduction by Frank McCourt reflects on the civil connection we feel with firefighters....The personalized spaces within the firehouses resonate as powerfully as the portraits of the firefighters themselves.”—The New York TimesOn September 11, 2001, more than 300 three New York City firefighters perished in the inferno and rubble of the World Trade Center. The Word Before the Powers: An Ethic of Preaching

Charles L. Campbell - 2002
Moving beyond a narrow focus on moral decision-making or social-issues sermons, Campbell argues that a particular ethic--nonviolent resistance--is inherent in the practice of preaching and shapes the moral life of the church. In the face of the powers, the fundamental ethical task of preaching involves building up the church as a community of resistance. Employing three dimensions of character ethics--vision, practices, and virtues--Campbell demonstrates the concrete ways in which preachers may undertake this task.

Working Identity: Unconventional Strategies for Reinventing Your Career

Herminia Ibarra - 2002
Feeling unfulfilled, burned out, or just plain unhappy with what we’re doing, we long to make that leap into the unknown. But we also hold on, white-knuckled, to the years of time and effort we’ve invested in our current profession. In this powerful book, Herminia Ibarra presents a new model for career reinvention that flies in the face of everything we’ve learned from "career experts." While common wisdom holds that we must first know what we want to do before we can act, Ibarra argues that this advice is backward. Knowing, she says, is the result of doing and experimenting. Career transition is not a straight path toward some predetermined identity, but a crooked journey along which we try on a host of "possible selves" we might become.Based on her in-depth research on professionals and managers in transition, Ibarra outlines an active process of career reinvention that leverages three ways of "working identity": experimenting with new professional activities, interacting in new networks of people, and making sense of what is happening to us in light of emerging possibilities.Through engrossing stories—from a literature professor turned stockbroker to an investment banker turned novelist—Ibarra reveals a set of guidelines that all successful reinventions share. She explores specific ways that hopeful career changers of any background can:Explore possible selvesCraft and execute "identity experiments"Create "small wins" that keep momentum goingSurvive the rocky period between career identitiesConnect with role models and mentors who can ease the transitionMake time for reflection—without missing out on windows of opportunityDecide when to abandon the old path in order to follow the newArrange new events into a coherent story of who we are becoming.A call to the dreamer in each of us, Working Identity explores the process for crafting a more fulfilling future. Where we end up may surprise us.

They say they'll give when they get rich. They say giving is painful because it demands sacrifice. They say a person who gives seriously is a saint or a sucker. They're wrong. The Art of Giving is a biography of Paul J. Meyer and how he is a role model for the next millennium. He doesn't give his surplus earnings: he sets giving targets, then works to meet them. It is also worthwhile reading for anyone, from any walk of life, who makes executive decisions. The leaders showcased here range from Frederick the Great to Norman Schwarzkopf. They include such diverse figures as Napoleon Bonaparte, commanders on both sides of the Civil War (William Tecumseh Sherman and Stonewall Jackson), German and American World War II generals (Rommel and Patton), a veteran of the Arab-Israeli wars (Moshe Dayan), and leaders from both sides of the Vietnam War (Vo Nguyen Giap and Harold Moore). All earned the respect and loyalty of those they led--and moved them to risk death. The practices of these commanders apply to any leadership situation, whether military, business, political, athletic, or other. Their words reveal techniques for anticipating the competition, leading through example, taking care of the troops, staying informed, turning bad luck to advantage, improvising, and making bold decisions.Leader after leader emphasizes the importance of up-front muddy boots leadership and reveals what it takes to persevere and win. Identifying a pattern of proven leadership, this book will benefit anyone who aspires to lead a country, a squadron, a company, or a basketball team. It is a unique distillation of two and a half centuries of military wisdom.

This powerful collection of classic books will equip you with the power and authority that belongs to believers in Christ. As a child of God, you have an abundant inheritance. Claim it now!Andrew Murray (1828-1917) was an amazingly prolific Christian writer. He lived and ministered as both a pastor and author from the towns and villages of South Africa. Some of Murray's earliest writings were written to provide nurture and guidance to Christians, whether young or old in the faith; they were actually an extension of his pastoral work. Once books such as Abide in Christ, Like Christ, and With Christ in the School of Prayer were written, Murray became widely known, and new books from his pen were awaited with great eagerness throughout the world. He wrote to give daily practical help to many of the people inhis congregation who lived out in the farming communities and could only come into town for church services on rare occasions. As he wrote these books of instruction, Murray adopted the practice of placing many of his more devotional books into thirty-one separate readings to correspond with the days of the month. At the age of seventy-eight, Murray resigned from the pastorate and devoted most of his time to his manuscripts. He continued to write profusely, moving from one book to the next with an intensity of purpose and a zeal that few men of God have ever equaled. He often said of himself, rather humorously, that he was like a hen about to hatch an egg; he was restless and unhappy until he got the burden of the message off his mind. During these later years, after hearing of pocket-sized paperbacks, Andrew Murray immediately began to write books to be published in that fashion. He thought it was a splendid way to have the teachings of the Christian life at your fingertips, where they could be carried around and read at any time of the day. One source has said of Andrew Murray that his prolific style possesses the strength and eloquence that are born of deep earnestness and a sense of the solemnity of the issues of the Christian life. Nearly every page reveals an intensity of purpose and appeal that stirs men to the depths of their souls. Murray moves the emotions, searches the conscience, and reveals the sins and shortcomings of many of us with a love and hope born out of an intimate knowledge of the mercy and faithfulness of God. For Andrew Murray prayer was considered our personal home base from which we live our Christian lives and extend ourselves to others. During his lateryears, the vital necessity of unceasing prayer in the spiritual life came to the forefront of Andrew Murray's teachings. It was then that he revealed the secret treasures of his heart concerning a life of persistent and believing prayer. Countless persons the world over have hailed Andrew Murray as their spiritual father and given credit for much of their Christian growth to the influence of his priceless devotional books.

Redefining the Strong-Willed Woman: How to Effectively Use Your Strong Will for God

Cynthia Ulrich Tobias - 2002
An appreciation and understanding of strong-willed women—whose nature can glorify Jesus Christ. Many strong-willed women have secretly wondered if there was something wrong with them, if perhaps they just weren’t trying hard enough to be good conforming Christian women. Cynthia Tobias provides a compelling and practical way for strong-willed women to come together and identify themselves to each other and the world. In Christian circles, women are often reluctant to come right out and say they are strong willed and proud of it. They have somehow been persuaded that God wouldn’t approve of a woman who is so—well, nonconforming. After all, isn’t the virtuous woman supposed to be quiet, holy, and subservient? Tobias reminds readers that the single most important thing they will ever do is accept Jesus Christ as their personal Savior. But beyond that, God gave each person their unique personality, gifts, learning styles, passions, and desires. He created and designed each person before they were even born. Whether readers sees themselves as strong willed women—or just need to understand the strong-willed women in their life—Redefining the Strong-Willed Woman provides powerful insight and encouragement for all types of women and men.

Spheres of Authority: Apostles in Today's Church

C. Peter Wagner - 2002
This wineskin, known as the New Apostolic Reformation, is the most radical change in the way of doing church since the Protestant Reformation five centuries ago. The central focus of this Reformation is the resurgence of apostles in the church. In this book, C. Peter Wagner, a widely recognized authority on church leadership and himself an apostle, takes a revealing look at the subject of apostles that is sure to satisfy many questions of both layperson and scholar alike including: · What is the definition of an "apostle"? · What are the characteristics of a real apostle? · Isn't it prideful to call oneself an apostle? · What does an apostle do? · What is appropriate apostolic authority? · What are the different spheres of authority an apostle may have? · What role do apostles have in transforming cities and territories? Using biblically sound theology, historical anecdotes, and personal experience, Wagner brings a new level of clarity to a subject that is easily misunderstood. This book is an important tool in both understanding and entering into the new wineskin God has given the church.

The Set-Up-To-Fail Syndrome: How Good Managers Cause Great People to Fail

Jean-François Manzoni - 2002
You decide to oversee that person's work more closely. After all, if your direct reports aren't delivering, it's your head that will roll. To further your frustration, the more you 'help', the worse the employee's performance becomes. What's going on? In this eye-opening book, leadership experts Jean-Francois Manzoni and Jean-Louis Barsoux expose a disturbing and surprisingly rampant phenomenon. While common wisdom assumes that so-called poor performers fail in spite of their boss' best efforts, this book demonstrates exactly the opposite. In many cases, a boss' attitudes and behaviors actually cause or 'set up' certain individuals - including those with great potential - to fail. Based on ten years of study into boss-subordinate relationships, Manzoni and Barsoux show that this Set-Up-to-Fail Syndrome is not confined to relationships with the proverbial 'boss from hell'. Even respected leaders - whether CEOs, teachers, or coaches - get caught up in it. The problem stems from the fact that while most managers empower and encourage star performers, they tend to micromanage and control perceived 'weaker' performers in ways that stifle self-confidence and drive. The unwitting result: the latter group lives down to expectations, rather than living up to its true potential. The cost of the Syndrome, say Manzoni and Barsoux, goes well beyond the lost productivity of a few individuals. It also threatens to derail careers, takes a heavy toll on morale, and hampers overall organizational results. Through dozens of interviews, illustrative stories, and compelling research, they show how readers can: determine whether they are involved in a set-up-to-fail dynamic, Recognize the mental biases that cause bosses to trigger the cycle; understand how subordinates contribute to fueling the problem; take specific steps to interrupt the cycle through proactive interventions; and, prevent the Syndrome altogether by managing relationships differently. For anyone with influence on an individual's potential, this book offers powerful ways to improve performance - and quality of life - in any organizational setting. Jean-Francois Manzoni is Associate Professor of Management and founding director of the research initiative on High Performance Organizations at INSEAD in Fontainebleau, France. Jean-Louis Barsoux is a Senior Research Fellow at INSEAD.
